Medical History/
Occupational Profile

We take a detailed look at the client's medical history to gain a deeper understanding of their physical and emotional conditions.

Analysis of Occupational Performance

- Select appropriate assessments

- We evaluate the client's abilities, including motor skills and problem-solving, to ascertain their strengths and challenges.

Synthesis of Evaluation Process

- Environmental factors, such as work and living conditions, are assessed to identify potential barriers to the client's ability to participate in meaningful activities.

- Interpreting the assessment data to identify supports and hindrances to occupational performance

Intervention Planning

Develop Plan/
Setting Meaningful Goals

- Working with the students to identify their personal goals, then develop specific, measurable, and achievable objectives that align with their occupational needs.

- Decide on methods for service delivery

Discharge Planning

Consider potential discharge needs and plans.

Team Collaboration

Working closely with the student, family, and the IEP team to ensure a collaborative approach to meeting the student's needs.

Intervention Implementation

Building Rapport

A therapeutic relationship is built between the client and the therapist to establish trust and rapport.

Focusing on Skills

Delivering skilled services to teach and practice new skills and adaptations directly related to students' annual goals.

Measuring Progress

Address newly identified challenges and make appropriate adaptions to ensure progress.

Intervention Review

Reevaluate

Reevaluate the plan and how it is implemented relative to achieving outcomes.
